New Clients

To become a new client, please complete the form below or contact the office at 229-233-8009 to activate the patient portal. When the portal is activated, you will receive an email welcoming you to the patient portal.

New Patient Referral Form

Once you are in the portal, you will be given these instructions:

New Patient Portal Instructions

  1. Register for the patient portal and log in. On the Dashboard, you'll see my tasks list of forms waiting for you — please complete all of them.
  2. Click your username in the upper right corner to open the drop-down menu, then select Profile and enter your demographic information.
  3. Go to Messages and upload photos of the front and back of your insurance card for our records. You're also welcome to send any additional documents or records you'd like us to have. Example: previous testing reports, treatment records, or court documents.
  4. Once you've finished these steps, give us a call at (229) 233-8009 and we'll get your appointment scheduled.
