Completing the referral form is the first step. Once submitted, our office will review your information and open our patient portal for you to complete the needed intake forms.
Step 1
Complete the referral form with as much detail as possible so we can understand your needs.
Step 2
Our office will review your submission and send you an invite to our HIPAA-compliant patient portal.
Step 3
Complete any requested intake paperwork, upload insurance cards (front and back), and any required background information.
Step 4
Once completed, please call our office to schedule your first appointment.
